Radiculopathy
Overview
What Is Radiculopathy?
Radiculopathy refers to pain, numbness, or weakness caused by irritation or compression of a spinal nerve root as it exits the spine. The spinal cord sends nerve roots through openings between the vertebrae, and these nerves travel into the arms, chest, or legs to control sensation and movement. When a nerve root becomes compressed or inflamed, the symptoms can radiate along the path of that nerve.
Radiculopathy can occur in different regions of the spine. Cervical radiculopathy affects the neck and may cause symptoms in the shoulders or arms, while lumbar radiculopathy often causes pain that travels down the leg, commonly referred to as sciatica. According to the Cleveland Clinic, radiculopathy occurs when a nerve root becomes pinched or inflamed, resulting in pain, tingling, numbness, or weakness along the nerve pathway.

Expert Analysis
Understanding Radiculopathy
The nerves exiting the spine are responsible for transmitting signals between the brain and the rest of the body. Each nerve root corresponds to a specific region of the body, meaning that irritation at a particular spinal level can produce symptoms in a predictable pattern. For example, a nerve compressed in the lower back may cause pain traveling down the buttock and leg, while a nerve affected in the neck may cause symptoms in the arm or hand.
Patients with radiculopathy often experience sharp, shooting, or burning pain that radiates away from the spine, along with numbness, tingling, or muscle weakness in the affected limb. The pain may worsen with certain movements, prolonged sitting, or activities that place pressure on the spine. If left untreated, ongoing nerve compression can lead to chronic pain, reduced mobility, and in some cases progressive nerve damage.

Triggers
Radiculopathy typically occurs when something places pressure on a spinal nerve root. One of the most common causes is a disc herniation or disc bulge, where spinal disc material presses against nearby nerves. Degenerative changes in the spine, such as facet joint arthritis or spinal stenosis, may also narrow the spaces where nerves exit the spine and lead to nerve compression.
Other contributing factors include spinal injuries, repetitive strain, poor posture, and age-related disc degeneration. As discs lose hydration and height over time, the surrounding structures may place additional pressure on the nerves. The National Institute of Neurological Disorders and Stroke notes that radiculopathy commonly occurs when nerve roots become compressed by disc problems, bone spurs, or narrowing of the spinal canal.

Differential Diagnosis
Symptoms of radiculopathy can resemble several other conditions, which makes accurate diagnosis essential. Patients may experience symptoms similar to peripheral nerve entrapments such as carpal tunnel syndrome, piriformis syndrome, sacroiliac joint dysfunction, muscle strain, or vascular conditions affecting the limbs. Because these conditions can produce similar pain patterns, identifying the true source of symptoms is critical.
At Compass Pain and Wellness, we prioritize precise diagnosis before beginning treatment. Our board-certified physicians use advanced diagnostic tools including flexion and extension X-rays, structure-specific diagnostic ultrasound exams, MRI imaging, nerve conduction velocity testing, bloodwork, and targeted diagnostic injections to determine exactly where the nerve irritation is occurring. By carefully identifying the affected nerve and the structure causing compression, we can design a treatment plan tailored to the underlying problem.
Solutions
Treating radiculopathy requires reducing nerve irritation while addressing the structural issues that are causing nerve compression. At Compass Pain and Wellness, we create individualized treatment plans that combine advanced interventional procedures, regenerative medicine, and supportive therapies designed to calm inflamed nerves and restore normal spinal function.
Treatment options may include:

What Makes Compass Different
Radiculopathy can be frustrating for patients because nerve pain is often treated symptomatically without identifying the exact source of compression. At Compass Pain and Wellness, our care begins with world-class diagnostic medicine. Our physicians take the time to listen carefully to each patient’s history and use advanced imaging and diagnostic techniques to pinpoint the precise location and cause of nerve irritation.
Once the diagnosis is established, we combine advanced regenerative medicine with a multidisciplinary treatment strategy to promote healing and long-term recovery. Treatments such as platelet-rich plasma and cellular therapy are delivered under precise ultrasound and fluoroscopic guidance to stimulate structural repair. Our integrated team—including physical therapy, chiropractic care, acupuncture, massage therapy, and advanced technologies like photobiomodulation, PEMF, and shockwave therapy—works together to support each patient’s recovery and restore optimal function.
