We’re located in a freestanding building, just west of downtown in the Goose Hollow Neighborhood, with easy access from both 405 and Hwy 26.
Parking:
We have a dedicated parking lot attached to the building with plenty of parking for all our patients.
Dr. Randolph Cribbs, MD
Dr. Randolph Cribbs, M.D. is a former Air Force general surgeon and flight surgeon. He received his undergraduate degrees in Physics and History from Stanford University and went on to earn his Medical Degree from the Pennsylvania State University College of Medicine. He has published a range of scientific papers in general surgery, pediatric surgery and wound care research, and received a First Prize award for his work in pediatric wound care. Dr. Cribbs has spent the past 15 years in urgent care and occupational medicine, devoted to helping patients receive the care they need to be able to resume their preferred work, home and leisure activities. In his spare time, he enjoys training and competing in endurance sports, including nine marathons and fifteen triathlons completed. He and his wife Cathy have six children and are now “empty nesters” with their Golden Doodle, Ricky. They are expecting their first grandchild in January 2025.

Dr. Carl Balog, MD
Dr. Carl Balog is a board-certified specialist in anesthesiology and pain management with a detailed, empathetic approach to patient care. He invests time and attention with every patient to discuss each of their treatment options and select the right solution for them. Dr. Balog completed his residency in Anesthesiology at UCSF, and another residency in Internal Medicine at Yale University. He became triple-board certified in 2014, and since, has leveraged his diverse medical training to ensure all aspects of a patient’s health are addressed to eliminate their pain and restore their maximum function.
Beyond his clinical work, Dr. Balog has contributed to the field through his role as Medical Director at Providence, his service on the ASIPP Pain Physician editorial board, and his leadership in guideline development. His professional contributions underscore his dedication to advancing the standard of care in pain management.

Jamin Rak, LMT
Jamin Rak is a licensed and board-certified massage therapist. He graduated in 1998 from East-West College in Portland Oregon and started his private practice, working in multidisciplinary clinics alongside physical therapy and chiropractic services. In 2005 Jamin combined his clinical experience in the field of pain science and rehabilitation to found Portland Massage Therapy, a center of excellence for the advancement of massage as a treatment for acute and chronic pain issues. He continues to increase his clinical knowledge with certifications in Advanced Myofascial Release Techniques and Orthopedic Assessment and Treatment.
Jamin is on the board for the Oregon Chapter of the American Massage Therapy Association where he is the Education & Conference Chairman. He was awarded the Meritorious Award for his service organizing statewide educational conventions and representing Oregon Licensed Massage Therapists as the elected delegate at national conventions.

Dr. Chase Gentry, DC
Dr. Chase Gentry’s first contact with chiropractic treatment was as a patient after being injured playing basketball in High School. He experienced first-hand the relief that was provided from chiropractic adjustments. This sparked a lifelong interest in helping others find the same relief that eventually led him to become a chiropractor himself.
Dr. Gentry attended Utah State University. He moved to Portland Oregon in 2009 and received his Doctor of Chiropractic degree from University of Western States. During this time, he also completed certification from the Spine Research Institute of San Diego where he studied whiplash traumatology, auto collision forensics, and traumatic brain injuries. While enrolled at the University of Western States, Dr. Gentry also participated in the Sports Medicine elective course, which focused on the evaluation and treatment of multiple sports related injuries.
In 2013, Dr. Gentry founded New Spine Chiropractic in Portland, Oregon, where he helped patients recover from motor vehicle collision injuries, sports injuries, and general aches and pains. He prides himself on treating every patient as if they are a member of his own family.
