Most conventional doctors still rely on a basic lipid panel that includes total cholesterol, LDL-C, HDL-C, and triglycerides. But these metrics are often incomplete, misleading, or entirely insufficient for assessing real cardiovascular risk.
Apolipoprotein B (ApoB)
A direct count of the total number of atherogenic particles.
LDL Particle Number and Size (LDL-P)
Small, dense particles are more dangerous.
Lipoprotein(a) [Lp(a)]
A genetic cholesterol particle that increases risk of heart attack and stroke by up to 4x.
Oxidized LDL (OxLDL)

Statins have been proven in countless studies to reduce cardiovascular risk by lowering LDL-C and stabilizing arterial plaque. But not everyone needs a statin—and not everyone tolerates them well.
At Compass, we don’t just hand out prescriptions. We evaluate whether you truly need a statin using ApoB levels, calcium scoring, genetic markers (like SLCO1B1), and LDL particle analysis. If a statin is appropriate, we select the lowest effective dose with the fewest side effects, often in combination with other therapies like ezetimibe or omega-3s.
We also provide support for those who experience side effects such as muscle pain or cognitive fog. This might include CoQ10 supplementation, switching to a different statin, or exploring non-statin therapies altogether.

We recognize the value of evidence-based nutraceuticals. Three we frequently incorporate are:
- Niacin (Vitamin B3): In therapeutic doses, niacin can lower LDL, reduce triglycerides, raise HDL, and uniquely lower Lp(a). We use extended-release or no-flush versions under close monitoring to minimize side effects and track liver enzymes.
- Soluble Fiber: Whether from psyllium husk, inulin, or beta-glucans (oat-based), soluble fiber helps bind bile acids and lower LDL. We often recommend a therapeutic dose of 10–15 grams per day, paired with dietary guidance to enhance lipid-lowering effects.
- Red Yeast Rice: A natural source of monacolin K (a compound similar to lovastatin), red yeast rice can help borderline patients achieve lipid reductions without full prescription statins. We use only pharmaceutical-grade versions, combined with CoQ10 and liver monitoring to ensure safety.

Your endocrine system plays a major role in lipid metabolism. At L2, we assess and optimize key hormones to support your cardiovascular health:
- Testosterone: Low T in men is associated with increased LDL, triglycerides, and metabolic syndrome. We offer BHRT when clinically indicated, improving lipid panels and vitality.
- Estrogen: In women, declining estrogen post-menopause correlates with reduced HDL and worsened lipid profiles. Bioidentical estrogen therapy may improve lipid parameters and vascular tone.
- Thyroid function: Even subclinical hypothyroidism can elevate LDL and ApoB. We assess TSH, Free T3, Free T4, and reverse T3, and optimize thyroid hormone replacement as needed.
- Cortisol and insulin: Chronically elevated cortisol drives insulin resistance and dyslipidemia. We evaluate stress hormone patterns and insulin sensitivity (via fasting insulin, HOMA-IR, and adiponectin) and tailor solutions accordingly.
